TO OBTAIN A COPY OF YOUR MEDICAL RECORDS OR X-RAY IMAGES

Patients may obtain copies of medical records or radiology images by coming by the Health Information Management (HIM) Department to sign an authorization to release medical records. The Release of Information staff will assist you in completing the authorization and let you know when records will be available. Copies of medical records may be picked up in the HIM Department, and copies of radiology images may be picked up in the Radiology Department.

There may be a charge for copies of medical records. There is no charge for copies that are faxed or sent to another health care provider. The charge may be waived under certain circumstances if the request is small.

Call 254-2377 if you need information about obtaining copies of medical records or radiology images. Release of Information hours are from 8:00 a.m. until 4:30 p.m. Monday through Friday.
